Experts say chances of a deadly worldwide outbreak of flu are increasing. The United States Department of Health and Human Services is in the process of preparing to reduce the impact of a pandemic influenza in the United States. The U.S. government is working to secure an adequate supply of vaccine for pandemic flu. In case of an outbreak, vaccine will be distributed according to predetermined priorities. Determining these priorities is a
difficult task.

A web engagement dialogue was held on December 4-6, 2007.  It sought to obtain public input from people throughout the U.S. on the government’s ten pandemic influenza vaccination goals. Participants discussed this critical health issue with public health officials as well as shared and voted on recommendations for revisions to the proposed draft guidance.  You can review the discussion summaries linked to each day's agenda. Dialogue participants' comments and suggestions were received.  All are being considered, along with other information, as the government refines the proposed guidance for distributing a limited supply of vaccine at time of a pandemic in the United States.

Everyone is familiar with seasonal influenza. Outbreaks of seasonal influenza or “the flu” occur every year in the United States. These outbreaks usually occur in the winter and can infect anywhere from 5% to 20% of people. For the most part the virus which causes seasonal influenza doesn’t change that much from year to year. Yearly influenza vaccinations are the best way to prevent seasonal influenza.

Perhaps not as many people are familiar with pandemic influenza. Pandemic influenza is different from seasonal influenza. It occurs when there is major change in the influenza virus that doesn’t respond to seasonal influenza vaccines.

Unlike seasonal influenza, everyone may be susceptible to pandemic influenza. Pandemic influenza can also spread much quicker than seasonal influenza and sicken more people. Pandemic influenza also can cause much more serious illness than seasonal influenza.

The 1918 influenza pandemic was the most deadly event in U.S. history, killing 675,000 people. Although we don’t know when there will be another outbreak of pandemic influenza or how severe it will be, another pandemic is sure to occur. If the next pandemic was as severe as the one that occurred in 1918, it could cause as many as 2 million deaths in the U.S.

These public engagements will help the federal government understand the public's values and preferences and contribute to guidance on how to prioritize who should be vaccinated earlier and who should be vaccinated later.

Because pandemic influenza won’t respond to seasonal influenza vaccines, a new vaccine will have to be developed to protect people.

And when the vaccine becomes available, the amount that is made each month will be limited.

Therefore, a priority list will be needed to determine who gets vaccinated earlier and who gets vaccinated later.
